  4. Upload Aadhaar, Family Details & Address Proof.
  5. Pay service fee online using UPI/NetBanking.
  6. Receive SMS/Email confirmation instantly.
  7. Saral Seva verifies your name in PMJAY/SECC list.
  8. Ayushman Card generated or downloaded.
  9. Download card anytime from My Applications.

Eligibility

Who Can Apply: Families listed under SECC 2011, NFSA list or PMJAY approved list.

Department: Ayushman Bharat – PMJAY (National Health Authority)

Benefits: Free health insurance up to ₹5,00,000 per family per year.
